Annual Membership Dues:
$400 per DME Provider,
plus $100 per additional location

I want to apply as a Sponsor

Diamond Level Sponsorship:
$5,000 per year

Platimum Level Sponsorship:
$3,000 per year

Gold Level Sponsorship:
$1,500 per year

Silver Level Sponsorship:
$750 per year
